Weekly Scholastic Monitoring

Weekly Scholastic Monitoring form to be filled out weekly for students with a grade average of 70% or lower. Form is to be filled out on Friday at school and presented at each Saturday morning practice until grade average shows improvement. Form is only acceptable when filled in by a Teacher, Signed and Stamped by a Guidance Counselor or School Official and signed by the Parent or Guardian. Students who fail to turn in the form on Saturday mornings will receive disciplinary action to be determined by NBYSA staff.
